The BBC's leaders failed to see the seriousness of the misleading editing by Panorama of Donald Trump's speech.
They seemed to assume that because Trump refused to accept the result of an election and implicitly encouraged the mob to storm the Capitol, it did not matter if the BBC made it look as if he explicitly incited violence.
The BBC has not learned enough from its failings in reporting the case for the Iraq war, because it never really accepted the findings of the Hutton inquiry, despite the resignations of its chair and director general in 2004.
What it should have learned is that if there are errors in its reporting, it must admit them quickly and act to prevent similar mistakes in future.
Everything else is decoration. Yes, the BBC is mostly great and worth defending, as Alan Rusbridger argues. Yes, it holds itself to higher standards than most other media organisations, as Samir Shah, its chair, argued to MPs.
But it ought to hold itself to a higher standard still.
